CASE MANAGEMENT COORD Case Management Coord - Mon-Fri Days; Communicate with case managers of
Managed care organizations regarding level of care for billing purposes,
estimated length of covered days, residents' progress and dates when
clinical notes are due, upon admission and throughout the stay; Review
managed care billing that has been denied/rejected or otherwise paid in
amounts or at levels that do not agree with submitted claims. Follow up
with case managers and/or MSH personnel to pursue or receive the correct
payment amount; facilitate resident disenrollment from Managed Care
insurance programs for those residents transitioning from short term to
long term care. Provide information on alternative programs providing
coverage for long term care; Attend morning report and participate in
weekly in-house Discharge Planning meetings; all other duties as

DIRECTOR OF PATIENT SERVICES Director of Patient Services - The successful candidate will be
responsible for handling the day to day operations; prepares,
implements, monitors and manages an effective budget of $115K+ including
billing systems: establishes and maintains relationships with other
vendors home care agencies, suppliers including service companies and
transportation; selects, maintains and supervises qualified professional
nursing staff and establishes staffing patterns adequate to meet the
needs of the client base population; actively develops and pursues

seminars. Evaluates, fabricates and provides communication boards to
residents as needed; Attends Comprehensive Care Planning/Discharge
Planning/Rehabilitation Rounds and other meetings as needed/assigned.
Performs chart audits as needed/required; Participates in performance
improvement activities to determine and ensure compliance with
documentation and care standards. Requirements: Masters in Speech
Therapy, NYS Speech-Language Pathologist license or eligible, and
clinical certification required.
